Verify customer information on the form as an EverBright customer and confirm loan information (such as account number, account name, balance, term, etc) provided by Great America.

Reminder: If the original request was received through a Jira ticket, be sure to remove PPI information such as social security number, close the Jira ticket, and continue communication/response via personal EverBright email.

Once information is confirmed, complete the “Verified By” or “Authorized Signature” section of the form through PDF Editor (if not able to complete, contact Cassandra Mercado. Typically, information requested is but not limited to:

  • Name

  • Phone number

  • Signature

  • Title

  • Date
